PLEASE NOTE

  • Correct footware must be worn on the pitch at all times, STUDS are NOT permitted
  • All participants must bring a packed lunch and drink for each day of the camp
  • The club reserve the right to to ask any participant / guest to leave the event if their behaviour is considered unacceptable

PHOTO CONSENT*

I GIVE permission for the participant(s) to be involved in photographing/filming and for information about the partipant(s) to be used for purposes within Newry Olympic Hockey Club.

LOCO PARENTIS CONSENT*

I CONSENT that if an emergency medical situation arises, the club may act as loco parentis for the participant(s). If the need arises for administration of first aid and/or other medical treatment which is in the opinion of a qualified medical practitioner may be necessary. I also understand that in such circumstances that all reasonable steps are taken.

NEWSLETTERS*

I (the parent/carer or lead participant) would like to opt-in to Newry Olympic Hockey Club newsletters to be kept informed about events and club activities.

DATA CONSENT*

Newry Olympic Hockey Club will only use information gathered in relation to the youth camp that meets the specific responsibilities as set out in the General Data Protection Regulations.
